All coaches and Parent Reps listed on team rosters must register with Eastern New York for a risk management background check using the link above. Failure to do so will delay completion and return of rosters and passes.)
ENYYSA Advance Notification of Player Injury Form (pdf) (The league recommends that this form be filled out and sent to Eastern whenever a player has been injured during practice or while playing in a sanctioned event and those injuries are sufficient to need a trip to a doctor or emergency room. After the player, coach and or parent fills out this form, it must be signed by the team coach then sent directly to ENYYSA - and forward a copy to the Club Registrar for club files)
